Highway setback line and no access to USH 14 should be shown <br /> graphically on the face of the CSM. <br /> Prior to recording this docui nent please place the following DOT approval number on the face of the document: <br /> DOT 13-014-0040-99C. <br /> Before any public street o: road (or private driveway) connection to the state trunk highway, as indicated on an <br /> approved final land divisioi i document, can be constructed, a permit must be obtained as required under Section <br /> 86.07, Wisconsin Statutes.
