<br /> As of January 1,1996,all persons that are required to have a Commercial Drivers License(CDL)to operate a
<br /> Commercial Motor Vehicle(CMV)and anyone that will be performing a"safety sensitive function",as defined
<br /> in 49 CFR Part 382 will be required to participate in a USDOT approved controlled substance&alcohol use
<br /> and testing program. This program must include,but is not limited to,provisions covering: random,post
<br /> accident,reasonable suspicion,return-to-duty and follow-up drug&alcohol testing,along with a provision for
<br /> pre-employment drug testing.
<br /> Accordingly,Hoffman Construction Company is requesting that you provide us with certification that you have
<br /> implemented a drug&alcohol program that meets,or exceeds,the provisions of the Hoffman Construction
<br /> Policy. We require that any program you implement will continue in full force and effect the entire period that
<br /> you work for,or at the direction of Hoffman Construction Company.
<br /> Failure to provide suitable verification that you have implemented an approved drug&alcohol program, may
<br /> be grounds for Hoffman Construction Company not to consider you for work,until such time as you certify
<br /> that you are in compliance with the law.
