We refer the Committee to the statement of Rick <br /> Sanders of Speedway Sewer Service contained in Attachment D. <br /> 4)Mr. Sanders o f Speedway Sewer Service has indicated to both the Town Clerk and the <br /> Town Chairman that the septic system located at 4123 Twin Valley Road does not meet <br /> existing standards for approval. <br /> 5)The beauty sh 3p located at this address includes plumbing devices attached to the <br /> septic system. In addition, Mr. Paul Musser, Town of Middleton Assessor has confirmed <br /> that a toilet facility has been constructed in the basement of the residence. <br /> The construction of the plumbing facilities requires the issuance of a zoning permit <br /> and a building l permit.There is no record that the Kanes ever applied for or <br /> received either zoning Permit(s) from Dane County or building permit(s) from the <br /> Town of Middlk ton regarding the remodeling of the accessory building or the <br /> remodeling of die residence. <br /> 6)The Town req.iested Ms. Kane to produce copies of the building permit or proof of <br /> payment for buil ding permits on November 5, 1997, during the Plan Commission <br /> meeting. To datl,no information or documents have been forthcoming from the <br /> Kanes. <br /> 7)The Town of Middleton recently completed an inspection of the accessory building. <br /> The Kanes denie.I the Building Inspectors access to the residence for purposes of <br /> inspecting the ba cement area including the toilet fixtures. The results of the inspection are <br /> summarized in Attachment E. <br /> The Town of Middleton respectfully requests that the Zoning and Natural Resources <br /> Committee reconsider the approval of Conditional Use Permit#1462 based on the information <br /> contained in this memos indum and our previous correspondence.
