• <br /> Page Four: <br /> During operation and crushing, owner/applicant shall take <br /> all reasonable efforts to ;reduce and minimize dust and shall <br /> take all reasonable efforts to minimize the noise of the <br /> operation. <br /> 15. Site shall be used for agricultural lime and gravel removal <br /> only. No redi-mix, concrete or asphalt production or <br /> manufacturing processes are allowed. <br /> 16. Owner/applicant shall perform private testing of all wells <br /> with 1/4 mile of the site with 90 days of opening the site with <br /> copies of the tests being given to the owners and township. <br /> Owner/applicant shall test the wells within 1/4 mile of the site <br /> " yearly thereafter with the results being given to the <br /> residential owners and the Township. <br /> 17 . Dumping or burying hazardous waste is prohibited and shall <br /> void the CUP and owner/applicant shall be responsible for all <br /> clean up costs. <br /> 18 . The Town Board shall be permitted to inspect the site <br /> following each crushing operation. The owner/applicant shall <br /> notify the Township when the crushing operation .has terminated. <br /> The purpose of the Town Board inspection shall be for inspection <br /> of the size of the quarry and shall review compliance with the <br /> CUP conditions. <br /> 19 . Blasting shall be performed in "accordance with the <br /> Wisconsin Administrative Code, Wisconsin Statutes and other <br /> regulatory agencies. <br /> 20. Owner/applicant shall reclaim all disturbed areas in the <br /> permitted boundaries and return the area to farming and <br /> agricultural uses. All topsoil and overburden shall remain on <br /> site. <br /> The quarry area will have a one (1) percent slope cut and <br /> graded to offer adequate surface water movement and in all cases <br /> sloped up toward Koshkonong Road with a bottom line to intersect <br /> less that 22 . 5 degrees cut slope from all roads involved in the <br /> extraction site. The transmission line easements note in the <br /> attached map shall be protected to their maximum width with cut <br /> back and cover set at a slope of no greater that 22 . 5 degrees <br /> and covered with appropriate cover crops for harvest and <br /> livestock use. All areas shall be covered with a minimum of two <br /> feet of soil to make it available for return to agricultural <br /> uses. <br /> Final reclamation shall be completed within one (1) year <br /> following the termination of the mineral extraction or within <br /> one (1) year following the end of the conditional use permit. <br />
